早产儿发生新生儿坏死性小肠结肠炎的危险因素分析

目的:分析探讨早产儿发生新生儿坏死性小肠结肠炎(Neonatal necrotizing enterocolitis,NEC)的高危因素,寻找有效预测指标,为NEC的预防提供新的循证医学证据。方法:选取本院2016年12月~2024年8月的71例<37周的早产儿,根据是否发展为NEC,分为对照组及NEC组。对比2组早产儿一般资料、孕母临床资料、发生NEC前的基础疾病及治疗措施,入院24小时内血常规、生化结果,应用多因素Logistic回归分析早产儿发生NEC的独立危险因素。结果:SNAPPE-Ⅱ评分越高、白细胞计数越高、血钠水平减低、无创呼吸机使用天数≥7d是早产儿发生NEC的独立危险因素,P<0.05。结论:我们在早产儿诊治过程中,应重视SNAPPE-Ⅱ评分的应用,动态监测白细胞计数及血钠水平,评估呼吸支持的必要性,尽可能预防NEC的发生,早发现、早治疗,改善早产儿救治的整体预后。
