目的：探讨高通量透析联合盐酸纳呋拉啡治疗维持性血液透析患者慢性肾脏病相关瘙痒症（CKD-aP）的临床疗效及安全性。方法：选取2024年12月至2025年12月于凉山彝族自治州第二人民医院肾内科行维持性血液透析合并CKD-aP的患者73例，采用自身前后对照设计，所有患者在维持原有高通量透析方案基础上，加用盐酸纳呋拉啡口崩片2.5μg每晚睡前口服，疗程4周。以视觉模拟量表（VAS）和瘙痒最严重强度数值评定量表（WI-NRS）评估瘙痒严重程度，同时监测血清甲状旁腺激素（PTH）、β2-微球蛋白（β2-MG）、血磷等实验室指标及不良事件。结果：治疗4周后，患者VAS评分由基线（76.97±9.19）mm降至（58.95±8.43）mm（P<0.001），WI-NRS评分由基线（7.27±1.19）分降至（3.52±0.72）分（P<0.001）。血清PTH、β2-MG、血磷、白蛋白、hs-CRP水平均较治疗前显著下降（P均<0.05）。治疗4周后，血清钙水平较治疗前升高（P<0.001）。不良事件发生率为8.54%（7/73），主要为轻中度且可自行缓解。结论：高通量透析联合盐酸纳呋拉啡能有效改善血液透析患者的CKD-aP症状，起效迅速，安全性良好，为该人群提供了一种有效的治疗选择。
