新型手术显微镜图像采集处理系统在乳腺癌及甲状腺癌手术切缘快速判读中的应用

目的:评价手术显微镜图像采集处理系统(EndoSCell Scanner,ES;DDB-5)在乳腺癌及甲状腺癌手术中进行细胞级切缘判读的准确性、时效性及临床可行性。方法:前瞻性、单中心、自身对照研究。纳入2026年于佳木斯市肿瘤医院行手术的乳腺癌及甲状腺癌患者16例,共选取47个样本。所有样本均行ES、术中快速冰冻病理(Frozen section,FS)及石蜡病理(HE)检查。以HE为金标准,分析ES与FS的诊断效能、一致性及耗时差异。结果:ES诊断灵敏度100.0%,特异度96.9%,准确性97.9%,AUC为0.984,不劣于FS。ES与FS判读结果Kappa值为0.952,三者间Kappa值为0.935,提示极好一致性。ES均值2.57±1.00分钟,显著短于FS的23.19±8.48分钟(P<0.001),平均每样本节省约20.62分钟。结论:DDB-5在乳腺癌及甲状腺癌手术切缘判读中准确性与FS高度一致,判读速度显著提升,有望作为传统FS的有效补充。
