目的:明确相对脂肪量(Relative Fat Mass,RFM)与成人肺功能指标之间的关联。方法:采用美国国家健康与营养调查(NHANES)2007-2012年的横断面数据,纳入5615名20-64岁符合标准的参与者。通过多变量线性回归模型分析RFM与用力肺活量(FVC)、一秒用力呼气容积(FEV1)、峰值呼气流速(PEF)、25%-75%用力呼气流速(FEF25%-75%)及FEV1/FVC比值的相关性;运用平滑曲线拟合和阈值效应分析探索潜在非线性关系。结果:完全校正模型显示,RFM每增加1个单位,FVC、FEV1、PEF分别显著下降0.17 L(95%CI:-0.21~-0.13)、0.11 L(95%CI:-0.14~-0.08)和0.20 L/s(95%CI:-0.29~-0.11)(均P<0.001),而RFM与FEF25%-75%无显著相关性(P=0.645)。阈值效应分析发现,RFM与FEV1/FVC存在非线性关联(P非线性=0.9738),拐点为22.1043;当RFM<22.1043时,FEV1/FVC随RFM增加显著下降(β=-0.0046,95%CI:-0.0064~-0.0028),RFM>22.1043时呈微弱正相关但无统计学意义(β=0.0009,95%CI:0.0001~0.0017)。结论:较高的RFM与成人肺功能(FVC、FEV1、PEF)下降风险呈正相关,且存在阈值效应。
