全脊柱内镜辅助下腰椎融合术(Endoscopic Lumbar Interbody Fusion,Endo-LIF)是微创脊柱外科领域的革命性进展,将内镜精准可视化优势与传统融合技术结合。本综述系统阐述Endo-LIF技术分类(Endo-PLIF、Endo-TLIF、Endo-OLIF)、核心操作要点、适应症拓展(腰椎间盘突出症伴不稳、退变性滑脱I-II度、椎管狭窄、腰椎术后失败综合征等)、临床疗效(VAS、ODI显著改善率>70%,平均融合率>90%)、并发症管理(神经损伤<5%,硬膜撕裂<8%)及其独特优势(出血量<50ml,住院时间<4天)。证据表明,Endo-LIF在精准减压、保留脊柱稳定性结构、加速康复方面优势显著。然而,陡峭学习曲线、镜下融合操作难度、长期随访数据待完善等问题仍需关注。未来发展方向包括器械智能化(机器人/导航整合)、生物材料创新(3D打印融合器)、操作标准化及扩大适应症探索(退变性侧弯、OLIF翻修)。
